Click on items and people to talk to them, or pickup bits. Once you've had enough of the home island, click on the hot air balloon to go up in it, and choose and island. There's a tutorial island, which maybe best to start with if you're not comfortable with how to move the character etc..

When the game starts, you're on the home island. If you click hte rope under the hot air balloon, you'll be shown a screen with the islands all, you can then click on one to go there. I did the carrot challenge yesterday which was simple and relatively fun.
